Licensed Practical Nurse - All Shifts

Job Summary:

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team at North Canton Nursing & Rehab, a leading provider of exceptional care and services in North Canton, Ohio. Responsibilities:
  • Provide direct patient care to residents, including medication administration, wound care, and vital sign monitoring
  • Develop and implement individualized care plans in collaboration with the interdisciplinary care team
  • Monitor and report changes in resident condition to healthcare providers and family members
  • Maintain accurate and up-to-date records of resident care and services
  • Participate in quality improvement initiatives to improve patient outcomes and satisfaction
  • Collaborate with other departments, such as rehabilitation therapy and social services, to provide comprehensive care

Requirements:
  • Current LPN licensure in the state of Ohio
  • High school diploma or equivalent required
  • 1+ year of experience in a long-term care or rehabilitation setting preferred
