The Wrap Around Service Coordinator (WASC) will provide service coordination for youth with complex, multi-system needs, ages 0 through 21 and their families by implementing all phases of the High Fidelity Wrap Around Program. Will ensure fidelity to the program model, engage families, develop crisis and safety plans, complete Strengths, Needs and Culture Discovery, develop Wrap Around Teams, facilitate meetings, create Individualized Plan of Care, track outcomes and data.

Expect approximately 30%-40% face‑to‑face time with youth and families.

• Provide service coordination for youth ages 0–21 using the High Fidelity Wrap Around Program.
• Ensure fidelity to the model during all four phases.
• Maintain an average caseload of 12–18 families.
• Employ strategies to facilitate family engagement.
• Conduct crisis stabilization and safety planning.
• Conduct Strength, Needs and Culture Discovery.
• Conduct evidence‑based assessments such as CANS, CASii.
• Develop Wrap Around Team with family and supports.
• Ensure regular communication among team members and community agencies.
• Create and implement Initial and ongoing Plans of Care.
• Document service coordination activities.
• Familiarity with strength‑based therapeutic models.
• Assist in state and local reporting as required.
• Participate in supervision meetings with WA Supervisor and SCFC Executive Director.
• Attend scheduled meetings.

Bachelor’s Degree in Social Work or related field

* * high school diploma/equivalent with at least four years of experience working with high‑risk youth and families. Service coordination experience preferred. Must demonstrate completion of the High Fidelity Wrap Around Facilitator’s Training or willingness to complete it upon hire.

Must possess and maintain a valid driver’s license and insurability.

Knowledge of the High Fidelity Wrap Around program, service coordination, therapeutic treatment models, strength‑based models, resiliency principles, and community resources. Skill in problem analysis, solution implementation, computer applications (Microsoft Office, EHR, data systems), communication, organization, time management, collaboration, facilitation. Ability to develop working relationships, work independently, uphold integrity, coordinate services, resolve conflicts, analyze data, maintain documentation, follow instructions, communicate effectively, and work flexible hours.
