Experience:
The Wrap Around Service Coordinator (WASC) will be responsible for providing service coordination for youth with complex, multi‑system needs, ages 0 through 21 and their families by implementing all phases of the High Fidelity Wrap Around Program. Ensure fidelity to the program model. Engage families in the Wrap Around Program, develop crisis stabilization and safety plans, conduct Strengths, Needs and Culture Discovery, develop Wrap Around Teams by maximizing the involvement of both informal and formal supports, facilitate Wrap Around Team meetings, develop Individualized Plan of Care for each youth and their family and track outcomes and data.
The WASC will be expected to have 30%-40% face‑to‑face time with youth and their families.
Bachelor’s Degree in Social Work or related field or High School Diploma/equivalent with at least four years’ experience working with high-risk youth and their families. Service Coordination experience preferred. The successful candidate must demonstrate successful completion of the High Fidelity Wrap Around Facilitator’s Training or the ability to complete the required training quickly following hire. The training will be provided by Stark County Family Council.
Licenseor Certification Requirements
Must possess and maintain a valid Driver’s License and insurability.
Essential Functions of the Position- Provide service coordination for youth ages 0 through 21 using the High Fidelity Wrap Around Program
- Ensure fidelity to the model during all phases of the program
- Maintain an average caseload of 12‑18 families in differing phases
- Employ strategies to facilitate family engagement
- Conduct crisis stabilization and safety planning activities as necessary
- Conduct Strength, Needs and Culture Discovery with each family
- Conduct assessments (CANS, CASii, etc.) at different points during the Wrap Around process
- Develop Wrap Around Team with family, including informal and formal supports
- Ensure regular communication among team members and community agencies
- Create Initial Plan of Care
- Coordinate implementation of Plan of Care, track outcomes and adjust as necessary
- Identify and address gaps in services
- Document service coordination activities
- Familiarity with additional strength-based, therapeutic care models
- Assist the WA Supervisor/CR in completing State and Local reports
- Participate in supervision meetings with the WA Supervisor and Executive Director
- Attend regularly scheduled meetings
Performs other related duties as required.

Inherently Hazardous or Physically Demanding Working ConditionsThis position will involve meeting families in various settings such as residences, schools, libraries, etc., which may be less than desirable.
