• Make outbound and receive inbound calls with members and healthcare partners.
• Build positive relationships through compassionate, professional communication.
• Assist Tailored Care Managers by completing outreach, scheduling appointments, coordinating services, and documenting member interactions.
• Help connect members with healthcare providers, community resources, and essential support services.
• Maintain accurate documentation while meeting quality and productivity goals.

• Excellent communication and active listening skills
• A passion for providing exceptional customer service
• Comfortable working in a high-volume call center environment
• Strong computer and multitasking abilities
• Experience in healthcare, customer service, call centers, or care coordination is a plus
