Summary

As part of our residential program team, the Case Manager will ensure the client’s services plan is developed and implemented, and goals and responsibilities are understood by all parties involved. The Case Manager will advocate for the client to ensure services are identified and rendered throughout their time utilizing Sagamore’s services. The Case Manger will be the lead with organizing, conducting, and follow up regarding the Child and Family Team meetings.

Duties And Responsibilities – Child Treatment & Development
  • Lead Admission efforts and be the main point of contact with youth, families, and referring agencies.
  • Coordinate and facilitate Child and Family Team meetings.
  • Lead the development, implementation, and evaluation of individualized services plans.
  • Develop trust and mutual respect with youth/families and their team.
  • Assist team to identify needs and create a shared vision.
  • Assist teams in identifying, recruiting, and engaging natural supports.
  • Enable youth/family and team to take ownership of the Wraparound process.
  • Support youth/family in identifying their strengths and building upon them.
  • Identify barriers to full participation in the program service process and develop strategies to overcome them.
  • Provide support and advocacy for the treatment team.
  • Prepare summary documents and final reports.
  • Ensure that all required confidentiality and other compliance documents are current for each youth.
  • Educate families, providers, and community about the elements of the program and its processes.
  • Contribute to and support a positive, team-oriented work environment.
  • Complete documentation of services and treatment plans consistent with clinical and administrative policies and procedures.
  • Work in collaboration with direct care staff, supervisors, clinicians, and the family to build rapport and overcome challenges.
Administrative & Training
  • Attends all mandated state training as well as all Sagamore Services required training.
  • Attends other in-service training as required.
  • Completes required documentation in a timely manner, concisely and professionally.
  • Adhere to all of Anderson Health Services policies and procedures.
Other Duties
  • Performs other related duties as required and assigned by Cottage Manager or Program Director.
  • Must be able to work flexible schedule to meet the needs of families.
Qualifications Education/Experience
  • Bachelor’s Degree from an accredited university, in a Human Services related field;
    Master’s Degree preferred.
  • At least two years of experience providing services for children and families. Must have experience facilitating small groups, preferably with multiple system involvement required.
  • Experience with case management database administration preferred. Must have a knowledge of community resources, preferably experience with local resources. Experience working with or living with children who have complex needs. Must have the ability to respond effectively within and cope with an environment that can be extremely stressful on the mind and body. Position requires driving agency vehicles as well as transporting clients.
  • A valid NC driver’s license, at least three years of driving with a driver’s license and clean driving record are required.
  • All potential job candidates must pass a drug screening test and an extensive background check.
  • Staff must be at least 23 years of age to work with clients ages 12 – 17.
Physical Requirements

Will frequently perform a wide variety of physical tasks such as standing, walking, running, stooping, bending, climbing stairs, which requires strength, coordination, endurance, and agility. Staff must be agile enough to control and rapidly extricate him/herself and youths from buildings in case of an emergency. Persons with any medical problems such as joint problems, obesity, arthritis or cardiopulmonary conditions, where it would be unwise or not possible to engage in moderate physical exertion should not be considered.

This program does require staff to be able to maintain sufficient physical agility and stamina to intervene with uncooperative youths and must be able to exert the minimum necessary strength to restrain and subdue the youth if necessary. Staff must be capable of running a distance as required in the prevention of a youth absconding.
