RN Community Liaison; Homecare

Overview

Make a meaningful impact by connecting patients, providers, and communities to exceptional care as an RN Community Liaison with our Homecare Team. Novant Health seeks a relationship‑driven professional who is passionate about building trusted partnerships with physicians, healthcare facilities, and community organizations while expanding access to home health services. In this role you’ll serve as a key ambassador, educating referral sources, strengthening strategic relationships, and ensuring patients receive the right care at the right time.

• Develop and maintain strong relationships with physicians, practice groups, hospitals, skilled nursing facilities, rehabilitation centers, assisted living communities, and other referral partners to promote home health services.
• Serve as the primary liaison for referral sources by responding promptly to inquiries, addressing concerns, and delivering exceptional customer service.
• Educate healthcare providers and community partners on home health programs, patient eligibility, referral processes, and available services to increase awareness and appropriate referrals.
• Collaborate with clinical and operational teams to support patient transitions of care, strengthen referral networks, and drive growth while ensuring an outstanding patient and customer experience.

• Education:
  
  4‑Year/Bachelor’s Degree preferred. Graduate of an accredited school of nursing required.
• Experience:
  
  Two years of sales and/or marketing experience in a health‑related field preferred.
• Licensure/Certification:
  Current RN licensure in the appropriate state required. Valid driver’s license in the appropriate state required.
