Position Summary:
Provide, schedule and coordinate transportation for residents’ necessary medical appointments.
Provide personalized care and service, uphold the philosophy of assisted living, and promote the well-being of the residents.
Provide assistance and support with residents’ activities of daily living, and instrumental activities of daily living.
Provide support to the Resident Care Team.
Transportation Services:
Reschedule if needed.
Necessary paperwork shall include but not be limited to: list of current medications (MAR), copy of all insurance cards, necessary progress notes, physician visit notes, and physician order sheet.
More or less paperwork may be required based on the nature of the physician visit.
Memory Care residents should not be left unattended at any time.
Additional support staff needed for appointments will be pre-approved by supervisor as needed.
Assist the resident in communicating concerns or problems to the Dr.Also communicate to the Dr. any concerns of the facility via the Physician/Facility Communication Form, if the resident is unable to voice concerns.
Discuss need for appointments with Care Coordinators.
Assist with picking up medications, delivering specimens to lab and/or doctor’s offices.
Break down charts after resident is discharged.
Maintain proper filing and storage of charts per regulations.
Ensure residents have adequate supplies at all times.
- Nurse Aide 1 or willing to train within 6 months
- Adult Care Home Medication Aide certification
- Experience in care giving to the older generation
- Ability to read, write and speak English
- Desire to work with older adults
- Friendly, caring disposition
Job Requirements
To fulfill job requirements, employee must be able to perform key physical tasks for extended periods of time.
When supporting, lifting, or transferring residents, proper body mechanics and safety measures as prescribed by the residence must be used.
In a typical eight hour shift the employee may be asked to:
- Stand/walk up to 8 hours per day
- Sit up to 2 hours per day
- Frequently support up to 70 pounds
- Occasionally lift/carry up to 15 pounds
- Frequently kneel, bend, reach and carry
- Secure proper assistance for transferring and lifting of residents as needed.
