Key Responsibilities of the RN – Home Health – Travel
- Provide high-quality home health and hospice care
- Manage a caseload of approximately 5-6 patients per day
- Conduct patient assessments and develop care plans
- Coordinate with unit secretaries and techs as required
- Document patient care using the HCHB system
Requirements of the RN – Home Health – Travel
- Valid RN license
- Home health and hospice experience
- Prior experience in the Franklin/Macon county area preferred
- Reliable vehicle for patient visits
