Behavioral Health Coordinator

Position Summary This individual will provide behavioral health support to residents of long term care facilities under the supervision of a licensed professional. They may receive training in a broad range of assessments and interventions that will support treatment specified in individual care plans. Regional travel will be required. Essential Duties And Responsibilities The following duties and responsibilities generally reflect the expectations of this position but are not intended to be all inclusive, other duties may be assigned:

• Conduct assessments that include the behavioral and social needs of the patients
• Meet with families and facility staff to coordinate care as recommended by licensed professionals
• Assess the patient’s knowledge of their clinical condition and provide education and self-management support
• Provide outreach to clients who are having problems with treatment adherence
• Assist in the development of the individual goals and behavioral health treatment plan
• Coordinate with other agencies and community providers
• Maintain timely documentation of work
• Serve as an active member of the behavioral health treatment team

Education And/or Certifications

• Bachelor’s degree in Counseling, Social Work, Psychology or related field

Skills And Qualifications

• Experience in a psychiatric or long term care setting preferred
• Customer service experience preferred
• Ability and willingness to function independently and proactively in a long term care setting
• Effective use of electronic medical records and Microsoft software (teams, excel spreadsheets, etc)
• Skilled in written and verbal communication.
• Skilled in exercising a high degree of initiative, judgment and discretion.
• Ability to handle confidential and sensitive information.
