Deer Park Health & Rehabilitation is seeking a highly skilled and dedicated Speech Therapist PRN to join our dynamic therapy team. As a Speech Therapist PRN, you will play a vital role in evaluating, diagnosing, and treating residents with speech, language, voice, communication, and swallowing disorders.

Key Responsibilities- Conduct comprehensive evaluations to assess speech, language, communication, cognitive-linguistic, and swallowing abilities.
- Develop and implement individualized treatment plans tailored to residents' needs and goals.
- Provide therapy services including speech, language, voice, fluency, cognitive-communication, and swallowing interventions.
- Monitor and document residents' progress, making necessary adjustments to therapy plans.
- Collaborate with an interdisciplinary team including nursing, physical therapy, occupational therapy, and physicians to provide coordinated care.
- Educate residents, families, and caregivers on strategies to support communication, safe swallowing, and carryover of therapy techniques.
- Ensure compliance with facility policies, professional standards, and state/federal regulations.
- Graduate of an accredited Speech-Language Pathology program (Master's degree required).
- Current and valid state licensure as a Speech-Language Pathologist.
- Certificate of Clinical Competence (CCC-SLP) preferred.
- CPR certification required (or must obtain upon hire).
- Experience in a skilled nursing or long-term care setting preferred.
- Strong communication, problem-solving, and organizational skills.
