Qualifications
Bachelor’s degree in Social Work or other human services areas such as sociology, special education, rehabilitation counseling or psychology; state-issued credential (LSW, LCSW, etc)
At least two years of experience in long term care Excellent verbal and written communication skills
Experience in completion of MDS, CAA and CP, BIMS and PHQ9
Strong organizational skills as evidenced by the ability to multi-task
Have compassion for the elderly and for their families
Proven ability to work in a team environment
Demonstrated investigation skills related to customer complaints and other resident concerns
Competency in electronic charting, Point Click Care preferred
Strong working knowledge of Windows, Microsoft Outlook, Word, and Excel
