Qualifications- Bachelor's degree in Social Work or other human services areas such as sociology, special education, rehabilitation counseling or psychology;state-issued credential (LSW, LCSW, etc)
- At least two years of experience in long term care
- Excellent verbal and written communication skills
- Experience in completion of MDS, CAA and CP, BIMS and PHQ9
- Strong organizational skills as evidenced by the ability to multi-task
- Have compassion for the elderly and for their families
- Proven ability to work in a team environment
- Demonstrated investigation skills related to customer complaints and other resident concerns
- Competency in electronic charting, Point Click Care preferred
- Strong working knowledge of Windows, Microsoft Outlook, Word, and Excel
