Occupational Therapist - Home Health

Role: Home Health Occupational Therapist responsible for providing personalized rehabilitation care in patients' homes to enhance strength, mobility, and independence.

• Assess and develop therapeutic retraining programs with measurable goals.
• Administer and interpret diagnostic tests, collaborating with physicians and healthcare teams.
• Provide therapeutic treatments to improve or restore functional abilities.
• Review and adapt retraining plans aligned with behavioral and rehabilitation goals.
• Consult with vocational team members to create patient-specific vocational plans.
• Report patient progress, participate in discharge planning, and coordinate care activities.
• Design community reintegration and adaptive equipment solutions.
• Train staff and family members on therapy interventions, supervise occupational therapy assistants and aides, and document patient care thoroughly.

• Current, unrestricted OT licensure and CPR certification.
• Valid driver's license, reliable transportation, and auto insurance.
• Preferred: minimum six months of occupational therapy and home health experience.

Pay per visit/unit rate ($54.00 - $76.00), with annual salary estimates between $85,400 and $117,500. Benefits include medical, dental, vision, 401(k), paid time off, disability, and life insurance.
