Care Manager; TCM; Greenville NC

ABOUT THE ROLE

Pathways to Life, Inc. is looking for a Tailored Care Manager to serve Medicaid members in the Greenville area who are living with serious mental health conditions and substance use disorders. This is a whole-person, community-based role: you'll be the point person who helps behavioral health treatment, medical care, housing, transportation, and benefits work together as a single, coordinated plan rather than a set of disconnected services.

We're looking for someone organized, steady under pressure, and genuinely invested in advocacy and long-term recovery; someone who can build trust with members while also navigating the practical realities of the healthcare and social service systems they depend on.

• Behavioral Health Care Coordination

• Develop and maintain person-centered care plans

• Coordinate behavioral health services, including outpatient therapy, psychiatric care, substance use treatment, MAT, Peer Support, ACTT, CST, and residential services

• Monitor member engagement and treatment participation, and identify gaps or barriers to care

• Support members through transitions following psychiatric hospitalization, detox, or residential treatment

• Help members access Medicaid-covered services and understand their options

• Guide individuals through healthcare systems and community resources

• Support applications for benefits, housing, transportation, and social services

• Advocate for medically necessary services and continuity of care

• Conduct outreach to members at risk of disengaging from care or entering crisis

• Coordinate crisis response and follow-up care

• Monitor high-risk members to help reduce emergency department visits and inpatient admissions

• Participate in discharge planning and post-hospitalization follow-up

• Coordinate communication among providers, family members, hospitals, therapists, psychiatrists, social service agencies, and community supports

• Participate in treatment team meetings, staffing meetings, and interdisciplinary case discussions

• Build and maintain working relationships with internal and external providers

• Complete Medicaid-required documentation and assessments accurately and on time

• Maintain clear, thorough progress notes and care management records

• Track quality measures and keep care plans current

• Ensure compliance with NC Medicaid Tailored Care Management requirements

This role moves between the office, the community, and the phone. On any given day, you might be doing a mix of the following:

• Community and home visits

• Telehealth and phone outreach

• Wellness checks and crisis outreach

• Hospital discharge coordination

• Documentation and care plan reviews

• Provider collaboration meetings

• Housing and transportation coordination

• Member advocacy and resource navigation

Candidates must meet one of the following:

• A bachelor's degree in a Human Services field, plus at least two years of direct experience working with individuals affected by mental health disorders, substance use disorders, intellectual or developmental disabilities, traumatic brain injuries, or related high-need populations

• A bachelor's degree in a non-Human Services field, plus at least four years of the same direct experience described above
