• Conduct comprehensive initial evaluations in the home
• Assess functional mobility, strength, balance, and safety
• Develop individualized, evidence-based treatment plans that align with patient goals
• Therapeutic exercise
• Gait and transfer training
• Fall risk assessments
• Home safety recommendations
• Instruction in assistive device use
• Provide detailed education to patients and caregivers on home exercise programs and strategies to optimize independence and quality of life
• Document findings and interventions in an electronic medical record
• Coordinate care with an interdisciplinary team of nurses, occupational therapists, speech therapists, and other clinicians to ensure cohesive plan of care, timely communication, and strong patient outcomes
