Mill Creek Center for Nursing & Rehabilitation is seeking a compassionate and skilled Physical Therapist Assistant (PTA) to join our supportive and collaborative rehabilitation team. In this part-time role, you'll help residents improve strength, mobility, independence, and quality of life through personalized, hands-on care.

Position Details
  • Part-Time

  • Skilled Nursing / Long-Term Care

What You'll Do
  • Provide hands‑on therapy under the supervision of a licensed Physical Therapist

  • Carry out individualized treatment plans designed to meet each resident’s goals

  • Provide individual and group therapy as appropriate

  • Monitor resident progress and communicate changes with the therapy and nursing teams

  • Complete accurate and timely clinical documentation

  • Assist with discharge planning and resident/family education

  • Collaborate with residents, families, nursing staff, therapists, and other care team members

  • Participate in care conferences and facility initiatives

  • Promote resident safety, dignity, mobility, and independence

What We’re Looking For
  • Graduate of an accredited Physical Therapist Assistant program

  • Current North Carolina PTA license

  • Previous skilled nursing or long‑term care experience preferred, but not required

  • Strong communication, organization, and time‑management skills

  • Ability to work effectively as part of a multidisciplinary team

  • Compassionate, dependable, and resident‑focused approach

  • Ability to safely assist residents with mobility and therapeutic activities
