RN Case Manager - Macon and Jackson | Four Seasons
Base pay range: $29.41/hr - $41.09/hr
Location:
Travel to home/nursing home/assisted living patients in the Macon and Jackson areas.
Employment type:
Full-time (FT) / Part-time (PT) / PRN. Shift: 40 hours, Monday – Friday 8:00 AM – 4:30 PM with rotating holidays per business needs.

Summary:
Coordinates the nursing plan of care, provides direct nursing care, and instructs/supervises LPN and Hospice Aide staff in the delivery of nursing care to clients. Manages an assigned caseload of approximately 15 patients with varying acuity; caseloads may be co-managed or adjusted based on needs.
- Practice within the scope of license, certification or registration, as applicable, and as required by law and regulation
- Assess patients’ physical and mental health, including reactions to illnesses and treatments
- Record and report nursing assessment results
- Plan, initiate, deliver, and evaluate appropriate nursing acts
- Teach, assign, or supervise other personnel in implementing the treatment regimen
- Collaborate with other health care providers to determine an appropriate patient- and family-centered plan
- Implement prescribed treatment and pharmaceutical regimens
- Provide teaching and counseling about the patient’s health
- Report and record the plan for care, nursing care given, and patient responses
- Supervise, teach, and evaluate staff involved in nursing functions and services
- Maintain safe and effective nursing care, whether rendered directly or indirectly
- Willingness to care for patients with infectious disease processes (accommodations allowed)
- Knowledge of Federal, State and Local regulations and Medicare Conditions of Participation
- HIPAA privacy rules and regulations knowledge
- Experience with electronic medical records systems; basic data entry and strong organizational and communication skills
- Ability to work under pressure, take initiative, and coordinate with multiple departments
- Flexible hours as needed; team player; ability to plan, prioritize and manage multiple projects
- Proficiency in pain and symptom management and understanding of reimbursement and regulatory requirements
- Experience:
Two years of clinical experience preferred - Licensure:
Valid NC RN license; valid driver’s license and proof of automobile insurance
- Walking, standing, sitting, bending, reaching, twisting, crouching/squatting, kneeling
- Arm/hand dexterity; lifting/carrying up to 50 lbs frequently
