This role brings home‑based medical care to complex, chronic patients, helping older adults stay at home.

Compensation: $40,000 Student Loan Repayment or $20,000 Sign‑on Bonus for individuals who have not previously participated in this program.

• Practice in accordance with state laws governing advanced practice nursing or physician assistants.
• Establish and maintain effective relationships with patients, caregivers, legal representatives, IDT members, PCPs, collaborating physicians/providers, and facility staff.
• Perform initial comprehensive assessments on newly enrolled patients and provide ongoing care. Develop and update patient management care plans as needed.
• Meet with patients/representatives to review care plans; involve PCP or supervising physician when applicable.
• Complete follow‑up and post‑discharge assessments per SOP.
• Prescribe diagnostics and interventions to avoid unnecessary acute admissions.
• Perform nursing procedures including IV placement, blood draws, injections, Foley catheters, nasal packing, wound care, etc.; receive new hire and annual training.
• Consult with hospital, emergency or post‑acute teams upon patient transfer notifications.
• Educate patients/representatives on disease processes and self‑management; promote wellness.
• Provide counsel, support, and education to staff as appropriate.
• Participate in ongoing patient‑care and clinical excellence meetings.
• Implement HEDIS measure campaigns and other quality initiatives to ensure high standards of care.
• Be on‑call for caseload during business hours and participate in the clinical after‑hours on‑call rotation.
• Stay current on relevant medical and nursing research, technology, and related issues through continuing education, professional meetings and journal reviews.
• Perform related duties as necessary and other duties as assigned.
