Join Our Team as a Family Practice Nurse Practitioner and Make a Difference in Patient Care!

Are you a compassionate and dedicated Nurse Practitioner seeking a meaningful role in family health? Our organization is looking for a motivated Family Practice Nurse Practitioner to deliver high-quality in-home health assessments that improve patient outcomes. In this role, you'll have the opportunity to work in a collaborative environment, providing essential healthcare services to diverse populations, all while enjoying a flexible schedule and comprehensive support.

Required Skills:

  • Valid NC Medical License
  • Enrolled and Credentialed in Medicard
  • Experience performing health assessments and chronic condition reviews
  • Strong clinical judgment and attention to detail
  • Excellent communication and interpersonal skills
  • Ability to work independently and prioritize effectively
Nice to Have

Skills:
  • Prior experience in home health or community-based care
  • Familiarity with A1C testing procedures
  • Bilingual abilities to serve diverse populations
  • Knowledge of Electronic Health Record (EHR) systems
Preferred

Education and Experience:
  • Master’s Degree in Nursing (MSN) or higher
  • Minimum of 2 years of clinical practice as a Nurse Practitioner
  • Previous experience in family or primary care settings
Other Requirements:
  • Must be enrolled and credentialed in Medicard
  • Must possess an active NC Medical License
  • Willingness to travel within Mecklenburg County and Wake County
  • Reliable transportation and adherence to travel reimbursements guidelines
  • Commitment to completing ongoing education modules during work hours
  • Ability to work full-time (40 hours/week) or part-time (20 hours/week), Monday through Friday, 8 am to 5 pm, including a one-hour lunch break
  • Paid orientation and comprehensive benefits package, including weekly pay via direct deposit, paid sick time, medical benefits, and mileage reimbursement at IRS standard rate
