Telos Health Systems staffs Psychiatric Mental Health Nurse Practitioners (PMHNPs), and General Medicine Nurse Practitioners, to improve residents’ quality of life and overall well-being while simultaneously reducing return to hospital stays.

• Act as a Skilled Nursing Facility’s leading provider of internal medicine services.
• Collaborate with the interdisciplinary team and physicians, serving as a day‑to‑day clinical leader while helping oversee the care of all residents.
• Work with short‑ and long‑term residents, overseeing their day‑to‑day care with the assistance of nursing staff and guidance from medical directors and attending physicians.
• Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in direct management of acute and chronic illness and disease.
• Prescribe all necessary medications and treatments.
• Perform comprehensive physical examinations of patients.
• Develop and implement patient management policies and procedures.
• Facilitate referrals to other healthcare professionals and medical facilities.
• Communicate with attending physicians and/or specialists regarding patient care.
• Be comfortable and proficient with documentation and CPT coding.
• Conduct monthly stability visits for long‑term residents.
• Perform evaluations for patients exhibiting changes in condition.
• Advance care planning.

• Certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP).
• Active state license and DEA number.
• Minimum one‑year experience in a Skilled Nursing setting (preferred).
• Ability to build rapport with patients and their loved ones.
• Ability to thrive in a fast‑paced environment.
• Excellent written and verbal communication skills.
• Strong leadership experience with emphasis on teamwork in a healthcare setting.
• Master’s Degree (required).
