Position: Assisted Living, Memory Care, LPN 7p-7a
Location: Oldtown

Job Summary:

The Licensed Practical Nurse (LPN) administers nursing care in a skilled nursing community under the supervision of a registered nurse (RN) or another medical supervisor and operates under the direction of physician orders. Provides basic medical care including assessing resident condition and monitoring for changes in condition, wound care, administering medication, resident/caregiver education, obtain specimens per physician orders. Ensures the health, comfort, and safety of patients by assisting with bathing, feeding, and dressing.

Essential Job Functions:

Provides direct resident nursing care and supervises the Medication Aides (SNF), Medication Techs (ALF) and CNAs within their neighborhood or unit in accordance with established plans.

Acts in the capacity as a supervisor in a modified primary care setting.

Complies with evaluation, treatment, and documentation of residents within their assignment.

Maintains ongoing communications with physicians concerning resident care including obtaining and reporting lab results, x-ray results, physical assessment abnormalities, etc.

Completes required documentation in an accurate and timely manner.

Confirms Residents have received their scheduled showers and that ADL care is provided as expected.

Facilitates problem solving and open communications with the nursing staff.

Ensures effective interactions with patients and families.

Ensures compliance with resident rights policies and work with families and other staff, as needed, to resolve grievances.

Acts in compliance with Salemtowne, regulatory and professional standards and guidelines.

Accesses continuing education opportunities appropriate to discipline and responsibilities to maintain clinical competency.

Adheres to facility policies and procedures and participates in facility quality improvement and safety programs.

Participates in and/or provide in-service education sessions.

Supports Salemtowne Shines culture & philosophy.

Performs other duties as assigned.

Completes annual compliance and HIPAA training and exhibits behavior as set forth by the Code of Compliance in the performance of their duties.

Adheres to Salemtowne’s policies and procedures regarding the compliance program of the organization and is timely in reporting any possible concerns.

Follows all local, state, and federal regulations as they pertain to their position.

Adheres to the Salemtowne’s policies and procedures covering privacy and security in compliance with HIPAA regulations.

Participates in nurse on-call schedule per policy.

Very strong observational and assessment skills with regards to physical/social/spiritual conditions, coupled with ability to discern effective response / treatment.

Able to embrace uncommon methods if they will best suit elders’ needs.

Sensitive to changing medical needs and the nature of staff responses and takes appropriate actions regardless of whether the needs have been reported by others.

Ability to model person-centered practices and provide direction to other team members as needed.

Capable of facilitating a sense of team connection and participation.

Adept in advocating for needs.

Skilled at mediating conflicts concerning team members as needed.

Ability to provide clear directions, expectations, and feedback.

Capable of eliciting trust and cooperation from team members.
