Position Overview

The Licensed Practical Nurse (LPN/LVN) provides and documents skilled nursing care under the supervision of a Registered Nurse, following care plans and physician orders while maintaining confidentiality and professional boundaries.

Responsibilities
  • Deliver and coordinate quality patient care per physician orders
  • Observe, assess, and monitor patient condition continuously; report changes
  • Document all activities, assessments, nursing actions, responses, and coordination of care promptly
  • Participate in, implement, and update nursing care plans
  • Take appropriate nursing action based on assessment to achieve expected outcomes
  • Recognize changes in patient needs and implement interventions to prevent or reduce risk
  • Comply with Aveanna policies, state and federal regulations, accrediting bodies, and the Nurse Practice Act
  • Use infection control measures in accordance with OSHA standards
  • Educate patients and families about disease processes, self‑care techniques, and prevention strategies
  • Maintain knowledge of nursing competencies via education, CEUs, internal learning, and skill evaluations
Requirements
  • Graduate of an accredited school of nursing
  • Current, unrestricted state license as an LPN/LVN in the state of practice
  • Current CPR certification
  • Proficiency in clinical assessments, documentation, and compliance with nursing care policies and procedures
  • Current, unencumbered license (or compact license transferred within 90 days)
  • Current TB skin test within last 12 months
  • Reliable transportation
Preferred Qualifications
  • At least six months recent experience as an LPN/LVN in a clinical care setting
  • Home health experience
  • One year of pediatrics experience (South Carolina), one year experience under current nursing license (California), or one year experience as a licensed nurse (Louisiana) – as applicable

  • Physical and mental response to emergency situations in the home or during transport
