Experienced Nurse Practitioners are needed for home health risk assessments with a focus on providing high-quality, patient-centered care in the Chowan County area. This per diem opportunity allows flexibility in scheduling, with shifts available between Monday and Sunday. You have the freedom to create your own work schedule, making this role ideal for practitioners seeking autonomy and a dynamic work environment.
Assignments may involve travel to various communities throughout Chowan County.
- Flexible shifts available every day of the week
- Set your own work schedule
- Minimum hourly commitment on a weekly or bi-weekly basis
- Complete comprehensive annual home health risk assessments for Medicare Advantage members
- Review and evaluate patients' past and present medical history, conditions, and medications
- Educate patients on managing their medical conditions and the use of medications
- Identify gaps in care and recommend steps toward resolution
- No requirement to develop treatment plans or prescribe medications
- Active Nurse Practitioner license in North Carolina
- Board Certification through AANP or ANCC
- Current CPR certification (BLS)
- MSN degree from an accredited nursing program
- Minimum of 6 months recent NP experience is strongly preferred
