What You Will DoAct as the Skilled Nursing Facility’s leading provider of internal medicine services.
Collaborate with the facility’s interdisciplinary team and physicians, serving as a day-to-day clinical leader while helping oversee the care of all the facility’s residents.
Work with the entire facility’s patient population — short and long-term residents — overseeing their day-to-day care with assistance from the nursing staff and guidance from the medical directors and attending physicians.
Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease.
Prescribe all necessary medications and treatments.
Perform comprehensive physical examinations of patients.
Develop and implement patient management policies and procedures.
Facilitate referrals to other healthcare professionals and medical facilities.
Communicate with attending physicians and/or specialists regarding patient care.
Be comfortable and proficient with documentation and CPT coding.
Monthly stability visits for long term residents.
Evaluations for patients exhibiting changes in condition.
Advanced Care Planning.
What You Will NeedCertification as a Psychiatric Mental Health Nurse Practitioner (PMHNP).
Must have an active license and DEA #.
Previous Experience In Nursing Or Other Medical FieldsMinimum one-year experience in a Skilled Nursing setting (preferred).
Ability to build rapport with patients and their loved ones.
Ability to thrive in a fast-paced environment.
Excellent written and verbal communication skills.
Strong leadership experience with emphasis on teamwork in a healthcare setting.
Education:
Master's Degree Required.
