Mountain Valley Hospice is seeking a Registered Nurse (RN) to complete hospice admissions in skilled nursing and assisted living facilities throughout Forsyth County.
This role is focused on guiding patients, families, and facility staff through the transition to hospice—bringing clarity, confidence, and compassion to what can often be an emotional and uncertain time.
What This Role Looks Like
You will work Monday through Friday from 8:00 AM to 5:00 PM, completing hospice admissions in SNF and ALF settings across the Forsyth County service area.
In this role, you will:
- Complete in-person hospice admissions in facility settings
- Perform clinical assessments and determine hospice eligibility
- Educate patients, families, and facility staff on hospice services and expectations
- Coordinate with referral sources, physicians, and the interdisciplinary team
- Ensure accurate, timely documentation to support compliance and quality care
You will be working with patients facing serious and terminal illness, and at times may encounter patients who are actively declining or near end of life. This role requires both clinical judgment and the ability to communicate clearly and compassionately in sensitive situations.

Qualifications
- Active RN license (NC or compact state)
- Strong clinical assessment skills and attention to detail
- Excellent communication skills and comfort in facility-based settings
- Ability to work independently while staying connected to a team
- Hospice, admissions, or case management experience preferred
