JOB SUMMARY

The Weekend/Holiday On-Call Nurse/Clinical Nurse Liaison is a hybrid clinical and referral-facing role responsible for providing responsive after-hours hospice nursing support while serving as the weekend and holiday point of contact for designated hospital partners. This position supports patients and families through triage, symptom management, urgent visits, admissions, education, and coordination of care, while also maintaining strong relationships with hospital referral sources.

PRIMARY JOB DUTIES

  • Provides scheduled weekend and holiday on-call coverage.
  • Respond promptly to patient, family, caregiver, facility, and hospital calls during assigned coverage periods.
  • Provide telephone triage, clinical assessment, education, and symptom-management guidance consistent with the hospice plan of care and MD orders.
  • Make timely in-person visits when clinically indicated, including crisis visits, symptom management visits, admission visits, and death visits.
  • Serves as the weekend and holiday hospice liaison for assigned hospitals and designated referral partners.
  • Respond to new referrals promptly and communicate referral status to hospital referral partners.
  • Review clinical information, assess hospice eligibility indicators and collaborate with the hospice medical director to determine appropriateness for hospice.
  • Coordinate discharge needs, including transportation, medications, DME, supplies and hand-off to the admission team.
  • Complete all referral, admission, and visit documentation accurately in near real time by the end of the business day.
  • Maintains positive, professional relationships with assigned hospital partners and represent the organizations mission, values and service standards.

JOB SPECIFICATIONS

  • Education: Graduate of an approved school of nursing, either through an AD, Diploma, or BSN program.
  • Licensure /

    Certifications:

    Current license to practice professional nursing in the State of North Carolina / South Carolina.
  • Experience: Prefer hospice or home care or related oncology experience.
  • Essential Technical / Motor

    Skills:

    Hand/eye coordination in order to give injections, use computer, etc. Ability to speak clearly to communicate with patients, families, physicians and staff regarding patient’s condition and care. Must be able to communicate and be literate in the English language.
  • Interpersonal

    Skills:

    Ability to develop positive interaction with patients, families, physicians and staff in order to effectively care for the patients.
  • Essential

    Physical Requirements:

    Ability to lift and/or maneuver objects weighing at least 45 pounds or less in the assessment and implementation of patient care. Requires pushing, moving, lifting of patients.
  • Essential Mental Abilities: Must be able to assess a patient’s condition, formulate a Plan of Care, implement appropriate interventions to meet patient/family outcomes. Requires higher level of mental faculties accompanied by short and long-term memory.
  • Essential Sensory Requirements: Must be able to assess a patient’s condition, utilize sight to implement and evaluate a Plan of Care. Utilize hearing to auscultate lung sounds, bowel sounds, and effectively communicate with patients, families, physicians, and staff.
  • Exposure to Hazards: Frequent exposure to blood and body fluids, infectious diseases, needle puncture wounds. May be exposed to dangerous animals and traffic hazards while home visiting. May encounter patients and other situations, which present a potential threat to personal safety. May encounter temperature changes and weather extremes.

    - Hours of Work:
    RN weekend on-call begins at 5:00p.m. Friday - 8:00a.m. Monday. Holiday on-call hours are 8:00a.m. – 8:00a.m. on actual designated agency holidays. Commit to working 48 of the 52 weekends per year. Commit to working all recognized agency holidays. No back-to-back weekends off.
  • Driving: May be required to drive. Must have reliable transportation. Will be subject to the Safe Driver Policy.
  • Population Served: Physicians, RN’s, facility administrators, discharge planners, social workers, other health professionals, insurance specialists, general public, community leaders, and vendors. All patient populations from infants through geriatric with life limiting illness.
