Position: Licensed Practical Nurse (LPN)
Location: Hampstead

The Licensed Practical Nurse (LPN) scope of practice in all steps of the nursing process is limited and focused because, by law, it is a dependent and directed scope of practice. LPN practice requires assignment or delegation by and performance under the supervision, orders, or direction of a Registered Nurse (RN), Physician, or other person authorized by State law to provide the supervision.

LPNs implement health care plans developed by the RN and/or by any person authorized by State law to prescribe such a plan.

Responsibilities
  • Administer nursing procedures to residents in accordance with the resident’s care plan
  • Ensure that a thorough report regarding residents’ status is received and given on assigned unit
  • Make rounds with Physicians/FNP
  • Notify Physician/FNP of changes in a resident’s condition
  • Take orders, transcribe orders correctly to the MAR/TAR; carry out/administer orders appropriately
  • Assist with daily and PRN documentation on needs and conditions for all Residents
  • Take, record, and monitor vital signs, and addresses any resulting care needed
  • Provide personal care services as needed to residents: oral care, bathing, shaving, nail/ hair care, dressing, repositioning, ambulating, assisting with elimination needs, transferring to and from wheelchair, bed, etc.
  • Make frequent rounds on the assigned unit to ensure care is being properly provided and that the residents environment is clean, free from clutter and safety hazards
  • Manage staff under LPN supervision to ensure care and documentation is complete and appropriate
  • Communicate recommendations from Pharmacy Consultant, Registered Dietitian, consultations from outside Physicians, etc. to resident’s Physician;
    Takes and transcribes orders recommendations as indicated
  • Admit and discharges residents as indicated
  • Administer medications, treatments, and care as ordered to resident’s in a safe, appropriate manner
  • Communicate any changes in the resident’s clinical condition, acute episodes, Physician orders, etc. with resident’s responsible party
  • Address concerns and questions related to resident’s care in a timely manner
  • Assist with emergency care and transfers as indicated
  • Any other duties as assigned by the Director of Nursing/Designee within scope of practice
Qualifications/Requirements
  • Responsible for maintaining the current license to practice as an LPN in the state of North Carolina
  • Must be able to attest to meeting the Continuing Competence Requirements at the time of license renewal as outlined by the North Carolina Board of Nursing.
  • Ability to work with any staff, Physician, visitor, vendor, resident, and family member regardless of race, gender, disease process, lifestyle, religious, spiritual or cultural beliefs, or treatment
  • Attend facility mandatory in-services/staff meetings as posted/scheduled
  • Complete facility mandatory competencies in a timely manner
  • Covid vaccine or religious/medical exemption (Required)
Education
  • Graduate from an accredited nursing program
  • Current Licensed Practical Nurse licensed in the state of North Carolina or licensure via reciprocity
  • Certified in Adult CPR. Must have completed an American Heart Association approved course
