Job Overview

Matrix Medical Network is a national in‑home clinical network offering comprehensive health assessments. We serve people across the nation, delivering personalized care that improves health outcomes and lowers costs.

Responsibilities

During a visit that can last up to one hour, Matrix providers review and observe a member’s current health, medical history, medication adherence, social environment and other risks. This provides unmatched insight into a member’s overall health and well‑being.

  • Conduct adult/geriatric assessments that include medical history, diagnosis and treatment, health education, physician referrals, case‑management referrals, follow‑up, and documentation according to Matrix guidelines and protocols.
  • Administer and order point‑of‑care and lab‑based testing as needed.
  • Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
  • Collaborate with Primary Care Physician (PCP) on patient education and follow‑up as necessary.
  • Provide services at the member’s place of residence, which may be in their homes or in Skilled Nursing Facilities.

    Visits are scheduled from Sunday to Saturday, 8 am–7 pm, with the option to work day, evening, or weekend hours based on your availability.

    Earn income based on the number of assessments completed (average 20–100 visits per month). Monthly earnings range from $1,800 to $11,500, depending on state and assessment completion rates of $90–$115.

  • Additional licensure:
    Support for obtaining additional state licensure and credentialing in neighboring states if needed.
  • Full‑time transition: PRN providers can transition to FT roles with a sign‑on bonus.
Qualifications
  • Master’s degree or commensurate experience and successful completion of NP licensure.
  • Current NP licensure in state of practice, including prescription authority or the ability to obtain prescriptive authority as needed.
  • AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty.
  • Current BLS, ACLS or CPR certification.
  • Preferred: 1 year of NP experience; new graduates encouraged to apply.
  • Valid state driver’s license for state of residence.
  • Vehicle insurance coverage:
    Bodily Injury $100,000 per person / $300,000 per incident;
    Property Damage $50,000;
    Collision and comprehensive deductibles $1,000 or less.
  • Strong computer skills and familiarity with employee health/medical record software.
  • Excellent verbal and written communication skills.
