Job Summary

Provides, coordinates and directs the provision of home health nursing services and case management processes according to the assigned patient’s individualized plan of care based on agency policies and procedures, through the competent application of nursing process.

• Provide home health nursing services and case management per individualized patient care plans.
• Coordinate care with interdisciplinary team and community resources.
• Maintain accurate documentation and compliance with regulatory standards.
• Operate autonomously in patient homes and nursing facilities.

• RN license, 1 year RN experience required.
• Driver’s license, reliable transportation.
• Strong communication and organizational skills.
• Comfortable working independently in home and facility settings.

• $80,000-$85,000 Salaried + mileage and cell phone reimbursement
• Full benefits including 401k match and generous PTO package, 1st of month after hire
• Robust training and 1 year RN experience required, driver's license
• Monday - Friday 8am - 5pm, limited back up on call
• Nationwide Company/ Hometown Feel
• Territory will be determined to where patients and new hire live - potential Williamston, Gatesville, Sunbury, Windsor, NC
