Center Well Home Health seeks a compassionate nurse to deliver one-on-one care in patients' homes, under the supervision of a Clinical Manager. You will plan and execute personalized care, educate families, and coordinate with physicians to support recovery and independence.
Requirements include a current state nursing license, CPR, a valid driver's license with insurance, and at least two years of LPN/LVN experience in a clinical setting. Home health experience is preferred.
