Registered Nurse (RN) – Assertive Community Treatment Team
Easterseals PORT Health is seeking a full‑time Registered Nurse (RN) to join our Assertive Community Treatment (ACT) Team. In this role you will provide essential, community‑based care to adults living with serious mental health and substance use challenges.
Schedule:
Monday‑Friday with rotating on‑call responsibilities. Community travel is required.
Salary Range: $75,000 – $90,000 annually, based on experience.

- Administer and monitor psychotropic medications as prescribed.
- Conduct nursing assessments and track symptoms, side effects, and medical issues.
- Document all care and observations in the Welligent EMR.
- Partner with the team psychiatrist to update medication protocols and care plans.
- Participate in daily in‑person ACT Team meetings to ensure coordinated, person‑centered treatment.
- Provide crisis assessment and intervention as part of a rotating on‑call schedule.
- Deliver nursing care in clients’ homes and community settings.
- Educate individuals on health management, wellness, and medication adherence.
- Advocate for client choice and connect individuals with needed community resources.
To be successful on the ACT Team, you should be compassionate, flexible, and comfortable providing care in diverse community environments.
Required Qualifications- Active, unrestricted RN license in North Carolina.
- Experience in behavioral health or mental health (preferred).
- Comfort working in community‑based settings.
- Valid driver’s license, reliable transportation, and vehicle insurance.
- Strong communication, documentation, and relationship‑building abilities.
