Licensed Practical Nurse

Health Systems Management is seeking a Licensed Practical Nurse to support dialysis patients in an outpatient clinic setting. In this patient-first role, you'll assist with dialysis treatments, monitor vital signs, administer medications within LPN scope, and document care in the EHR. You'll collaborate with RNs, nephrologists, and technicians to maintain clinical quality, safety, and regulatory compliance. This position offers exposure to clinic operations, quality improvement, and chronic kidney disease management in a collaborative, growth-oriented environment.

Responsibilities

• Provide direct nursing care to dialysis patients under RN and physician supervision
• Monitor, document, and report vital signs, patient responses, and treatment outcomes
• Prepare dialysis stations, equipment, and supplies following infection control standards
• Administer prescribed medications and injections within LPN scope of practice
• Assist with patient education on treatment plans, medications, and home care
• Maintain accurate electronic health records and regulatory documentation
• Support quality improvement and safety initiatives in the clinic
• Collaborate with multidisciplinary team to optimize patient outcomes
• Ensure compliance with clinical protocols, CMS regulations, and clinic policies
