Team Health is seeking an experienced family nurse practitioner or physician assistant to help launch a new rural health clinic serving Stanly County, North Carolina. Clinic also offers integrated behavioral health services through dedicated behavioral health clinicians

This position is focused exclusively on primary care. Behavioral health counseling, psychiatric medication management, and medication-assisted treatment (MAT) services are provided by dedicated behavioral health specialists. The FNP or PA is not responsible for providing or overseeing behavioral health or MAT services.

What You’ll DoProvide comprehensive outpatient primary care for patients across the lifespan, including infants, children, adolescents, adults, and older adults

Diagnose and treat acute illnesses and injuries

Manage chronic medical conditions using evidence-based practices

Perform comprehensive patient evaluations, histories, and physical exams

Deliver preventive care, wellness visits, immunizations, and routine health maintenance

Perform Medicare annual wellness visits and age-appropriate screenings

Develop individualized treatment plans and coordinate care with specialists and community resources

Educate patients and families regarding disease prevention, healthy lifestyle choices, and chronic disease management

Collaborate with the medical director and interdisciplinary care team to deliver coordinated, patient-centered care Maintain accurate, timely, and compliant documentation within EHRYou’ll Be Supported By…Rural health clinic medical director

Additional FNP and/or PA Licensed clinical social worker

Psychiatric mental health nurse practitioner

Licensed practical nurses

Patient services representatives

Team Health’s operational and clinical leadership teams

Qualifications Required Board -certified family nurse practitioner or physician assistant

Active North Carolina license (or ability to obtain)
DEA registration (or ability to obtain)
Ability to provide comprehensive primary care across the lifespan

Preferred

Two or more years of outpatient primary care experience
