We are seeking a dedicated and passionate Licensed Practical Nurse (LPN) to join our skilled nursing team. As a Licensed Practical Nurse at our facility, you will play a vital role in providing high quality care, supporting our residents' health and well-being, and working closely with our team to ensure success!

Licensed Practical Nurse (LPN)

Qualifications:
  • Hold an active, unencumbered Licensed Practical Nurse (LPN) NC license or that of a compact state
    .
  • Demonstrate strong clinical skills, critical thinking, and attention to detail.
  • Communicate effectively and professionally with residents, families, and team members.
  • Previous experience in long-term or skilled nursing care preferred but not required.
Additional Duties:
  • Provide direct patient care, including administering medications, resident physical assessments, performing treatments, documentation, and monitoring vital signs
  • Provide wound care treatments as needed (wound care nurse in house weekdays to provide scheduled treatments)
  • Medication administration
  • Peripheral IV access
  • Admission assessments
  • Provide discharge education and instructions
  • Catheter insertion and removal
  • Conduct comprehensive assessments of patients' health status and develop individualized care plans
  • Collaborate with healthcare team members to coordinate and manage patient care
  • Blood draws for labs
  • Educate patients and their families on disease prevention, treatment options, and self-care techniques
  • Document patient information accurately and timely in electronic medical records (Point Click Care)
  • Adhere to all relevant medical protocols, standards, and regulations
  • Skilled in triage and prioritizing patient needs based on urgency
