- Assess and monitor patients' health conditions in their home environment
- Develop and implement individualized care plans

- Use the nursing process to assess, plan, implement, and evaluate patient care in the home setting
- Provide individualized care for adult and geriatric patients throughout the lifespan
- Manage a caseload and coordinate care with a multidisciplinary team
- Communicate information according to agency policies and discipline-specific guidelines
- Contribute to program effectiveness and maintain agency performance
- Organize work effectively and efficiently
- Maintain and adjust schedules to enhance agency performance
- Demonstrate agency mission, vision, values, and positive interpersonal relations
- Maintain and promote customer satisfaction
- Provide care and case management for patients in their homes
- Work variable Monday–Friday hours and holidays as needed
- Graduate of an accredited or approved school of nursing, either an AD, Diploma, or BSN program
- Current license to practice professional nursing in the State in which providing care (NC/SC)
- CPR certification required
- One year RN experience and a total of 2 or more years clinical experience is required
- Home health experience preferred
- Must be able to communicate and be literate in the English language
- Ability to transfer and/or maneuver objects weighing at least 50 pounds
- Valid driver’s license and an operational vehicle
- Ability to assess patient conditions, formulate care plans, select interventions, evaluate responses, and explain/teach patients
- Ability to prioritize duties, learn new skills and techniques, and use supportive services
- Ability to visually assess patients and utilize hearing for clinical assessment and communication
- Ability to manipulate patient care equipment and properly transfer and guard patients
Demonstrates expertise in patient assessment, individualized care planning, and case management for adult and geriatric patients in home health settings. Proficient in collaborating with multidisciplinary teams to enhance patient outcomes while maintaining agency performance and customer satisfaction.
