Job Summary:

Provide licensed nursing care to patients in a Long-Term Care or Skilled Nursing Facility setting. Ensure patient safety and comfort while adhering to established care plans and facility protocols.

Responsibilities:

• Administer medications and treatments according to physician orders and regulatory guidelines.
• Monitor patient conditions and report any changes or concerns to the healthcare team.
• Document patient care accurately and timely in electronic health records.
• Assist with activities of daily living and provide personal support to residents.
• Maintain a safe and clean environment, following infection control protocols.

Qualifications:

• Valid LVN or LPN license in the state of practice.
• Proven experience providing nursing care in LTC or SNF environments.
• Ability to follow detailed care plans and protocols.
• Strong interpersonal and communication skills.
• Must be fully vaccinated against COVID-19 or have an approved exemption.
