• Perform comprehensive assessments of high-risk members and develop individualized care plans.
• Collaborate with primary care providers to implement targeted interventions and ensure appropriate care levels.
• Identify and continuously reassess cost-effective and suitable levels of care for members.
• Coordinate non-medical support such as housing and transportation to promote treatment adherence.
• Manage care transitions by coordinating with physicians, social workers, discharge planners, and claims professionals.
• Engage specialty resources as necessary to optimize health outcomes.
• Document clinical, functional, and fiscal outcomes during treatment management.

• Monday - Friday, 8:00 AM - 5:00 PM, on-call for PRN every 5th week.
• On-site position in Elizabethtown, NC.
• Compliance and submission requirements applicable.

• Associates or Bachelor’s Degree in Nursing or related field.
• Current, unrestricted state Registered Nurse license.
• Case Management certification.
