Responsibilities

As a Foster Care Case Manager at Health Connect America, you'll play a critical role in supporting children and families within the foster care system—ensuring they receive the coordinated, compassionate care they need to thrive.

What You'll Do

As a Foster Care Case Manager, you'll oversee a caseload of children and families, ensuring services are well‑coordinated, plans are individualized, and every child’s needs are met with consistency and care. Your work will be relationship‑driven, detail‑oriented, and focused on long‑term outcomes. Day to day, you can expect to:

  • Develop and implement individualized treatment and service plans that support each child’s safety, stability, and well‑being
  • Coordinate medical, educational, behavioral health, and social services to ensure seamless, uninterrupted care
  • Serve as a central point of communication between foster parents, biological families, service providers, and internal teams
  • Facilitate and support healthy interactions between children, foster families, and birth families to promote positive outcomes
  • Monitor and continuously assess each child’s progress, adjusting care plans to meet evolving needs
  • Maintain accurate, timely documentation and case records in accordance with organizational and regulatory standards
  • Partner with foster parents to provide guidance, support, and resources that help create safe, nurturing home environments
  • Collaborate with internal teams and external agencies to ensure coordinated service delivery and compliance with all requirements
What to Expect

This role requires strong organization, adaptability, and the ability to manage multiple priorities in a dynamic environment. You’ll be working closely with a variety of stakeholders, often balancing urgent needs with long‑term planning. Your ability to communicate clearly, problem‑solve proactively, and maintain attention to detail will be key to success.

You’ll also play a role in supporting foster parent recruitment, assessment, and ongoing engagement—helping build a network of caregivers who are equipped to provide safe and supportive homes.

Supporting Quality & Compliance

In this role, you’ll ensure all services and documentation meet Health Connect America (HCA), state, and Council on Accreditation (COA) standards. You’ll be expected to maintain high‑quality work, meet required timelines, and actively contribute to continuous improvement efforts—ensuring the best possible outcomes for the children and families you serve.
