Advocate Health is seeking a Social Worker II in Wake Forest Baptist Medical Center's Nursing:
Care Coordination to support patients and families through complex medical and social challenges and ensure safe discharge planning. This 40-hour/week, full-time role emphasizes assessment, care coordination, referrals, and collaboration with a multidisciplinary team to improve patient outcomes and continuity of care.
Master’s in Social Work and clinical licensure are preferred.
