Position OverviewThe Social Worker position plays a vital role in ensuring safe, timely, and effective transitions of care for patients. This position supports patients and families by coordinating post-hospital services, addressing social and financial needs, and promoting continuity of care.

- Coordinate discharge planning for patients across all age groups
- Collaborate with physicians, nurses, and interdisciplinary teams to develop appropriate care plans
- Educate patients and families on post-discharge care, services, and available resources
- Connect patients with community agencies, home health services, and support programs
- Assist with financial, social, and placement needs
- Conduct follow‑up to ensure continuity and effectiveness of care plans
- Support efforts to reduce length of stay and improve patient outcomes
- Advocate for patients while supporting their emotional and social well‑being
- Education:
High School Diploma or equivalent – required - Preferred Education:
Bachelor’s degree in Social Work or related field - Experience:
Healthcare, case management, or social services experience preferred
- Strong communication and interpersonal skills
- Organizational and time management abilities
- Critical thinking and problem‑solving skills
- Compassionate, patient‑centered approach
- Ability to manage multiple priorities in a fast‑paced environment
- Compliance with all hospital policies, including vaccination requirements (approved exemptions considered)
- Successful completion of a criminal background check and pre‑employment drug screening (post-offer)
