Home Health Quality Assurance (QA) Registered Nurse

Responsibilities
  • Lead the agency's Quality Assessment and Performance Improvement (QAPI) program to ensure compliance with company standards, Medicare Conditions of Participation, and applicable federal and state regulations.
  • Conduct clinical record audits to evaluate documentation quality, regulatory compliance, and adherence to agency policies.
  • Review clinical documentation to identify trends, opportunities for improvement, and initiatives that enhance patient outcomes and quality of care.
  • Collect, analyze, and report quality and performance data to support agency goals and continuous improvement efforts.
  • Collaborate with Clinical Managers and agency leadership to develop and implement corrective action plans and quality improvement initiatives.
  • Provide education, coaching, and in-service training to clinical staff on documentation standards, regulatory updates, agency policies, and quality initiatives.
  • Serve as a clinical resource regarding home health regulations, accreditation standards, evidence-based practices, and quality improvement strategies.
  • Assist with survey readiness activities, regulatory audits, and compliance initiatives.
  • Participate in Clinical Management Team meetings and provide recommendations based on quality data, audit findings, and performance metrics.
  • Partner with leadership to review, develop, and update policies and procedures that promote regulatory compliance and clinical excellence.
  • Foster a culture of continuous quality improvement through education, mentoring, and ongoing performance monitoring.
  • Maintain compliance with HIPAA privacy and security regulations while safeguarding Protected Health Information (PHI).
Qualifications
  • Graduate of an accredited school of nursing with a current, unrestricted Registered Nurse (RN) license.
  • Bachelor's degree in Nursing (BSN) preferred.
  • Minimum of three (3) years of clinical nursing experience, including at least one (1) year of recent home health experience.
  • Experience in Quality Assurance (QA), Quality Assessment and Performance Improvement (QAPI), clinical auditing, or performance improvement activities preferred.
  • Knowledge of Medicare Conditions of Participation, state and federal regulations, accreditation standards, and home health best practices.
  • Strong understanding of OASIS documentation requirements and home health clinical documentation standards.
  • Excellent analytical, organizational, and problem-solving skills with exceptional attention to detail.
  • Proficiency with electronic medical record (EMR) systems, Microsoft Office Suite, and data analysis tools.
  • Outstanding written, verbal, and interpersonal communication skills with the ability to educate, mentor, and collaborate with clinical and non-clinical staff.
  • Ability to manage multiple priorities, adapt to changing regulations, and work independently while supporting interdisciplinary teams.
  • Demonstrated commitment to professionalism, continuous learning, and improving the quality of patient care.
  • Valid driver's license and current automobile liability insurance if travel is required for the position.
