Licensed Practical Nurse

Bold Age PACE seeks a compassionate Licensed Practical Nurse to support comprehensive care for older adults in our coordinated PACE program. In this full-time role, you'll provide direct nursing care, administer medications, support assessments, and document in the EHR while collaborating with an interdisciplinary team. Working across clinic, home, and community settings, you will help manage chronic conditions, educate participants and families, and promote safety and independence.
