Job Overview

Graham Healthcare Group is seeking a Home Health Physical Therapist for a job in North Chicago, Illinois. The position is full‑time, ongoing, 40 hours per week, 8‑hour shift, staff employment type. Residential Home Health and Hospice (“Residential”) is looking to add to its extraordinary care team. We believe outstanding care is best delivered in a team‑based environment, and home health physical therapists collaborate to provide management and delivery of patient care plans.

This role supports patients in Gurnee and surrounding areas and is eligible for a $20,000 total bonus potential, including a $10,000 first‑year success bonus and an additional $10,000 sign‑on bonus paid within the first 18 months of employment. Bonuses are awarded for good standing and meeting performance expectations.

• Bachelor’s degree or Master’s degree in Physical Therapy from an accredited college (required)
• Licensed Physical Therapist issued by the state in which you will be working
• Minimum of one year healthcare experience
• Current driver’s license and ability to drive approximately 20% of your day to patient locations
• Commitment to consistently meet critical deadlines for charting
• Self‑management of time and schedule
• Demonstrated experience with tablets, mobile phones, and EMR software
