Are you a Nurse Practitioner looking for an opportunity where you can provide meaningful patient care without nights, weekends, or hospital call? Chicagoland Infectious Disease is expanding our clinical team and seeking a Nurse Practitioner to provide care to medically complex patients in skilled nursing facilities throughout the Macomb area.

You do not need to be an Infectious Disease specialist to be successful in this role. We welcome experienced NPs with backgrounds in primary care, internal medicine, geriatrics, hospital medicine, skilled nursing, post-acute care, wound care, hospice, or other complex-care settings. This is an excellent opportunity for an NP who wants to expand their clinical expertise while enjoying greater flexibility and work-life balance.

What

You'll Do.

As a Nurse Practitioner, you will provide evaluation, treatment, and ongoing management of patients in skilled nursing facilities.

  • Conduct comprehensive patient assessments.
  • Diagnose and manage acute and chronic conditions.
  • Develop and implement individualized treatment plans.
  • Evaluate and manage infectious conditions.
  • Review laboratory and diagnostic results.
  • Prescribe medications and adjust treatment plans as appropriate.
  • Collaborate with physicians, nursing staff, facility leadership, and other members of the care team.
  • Provide education to patients, families, and healthcare professionals.
  • Monitor patient progress and clinical outcomes.
  • Document patient care accurately and timely.
  • Support evidence-based infection prevention and antimicrobial stewardship practices.

Infectious Disease experience is preferred, but not required.

Qualifications.
  • Active Nurse Practitioner license in Illinois.
  • Board certification as a Nurse Practitioner.
  • Current DEA registration or eligibility.
  • Current BLS certification.
  • Strong clinical assessment and decision-making skills.
  • Excellent communication and interpersonal skills.
  • Ability to work independently while collaborating effectively with physicians and facility teams.
  • Experience with electronic medical records.
  • SNF or long-term care experience is a plus.
