MDS Registered Nurse; RN PRN

The MDS Nurse is responsible for organizing, completing, and evaluating resident assessments and care plans to promote optimal resident functioning, ensuring compliance with state and federal regulations.

• Coordinate MDS, CAAS, and care planning processes upon resident admission.
• Conduct monthly triple checks for Medicare, managed care, and outpatient therapies.
• Observe, report, and document changes in residents’ physical and emotional conditions.
• Establish care plan dates within 7 days of assessments or reviews.
• Ensure nursing documentation aligns with care plans during treatments.
• Communicate assessment reference dates (ARD) with the interdisciplinary team.
• Interview residents and families for accurate assessments, and consult with physicians as needed.
• Participate in quality improvement programs and community inspections.
• Report and investigate resident abuse, neglect, or property misappropriation.
• Perform administrative duties including documentation and reporting.
• Mentor and support professional growth of other MDS nurses.
• Respond promptly to crises or assistance requests.

• Licensed Practical Nurse (LPN) or Registered Nurse (RN) with a current license.
• At least 2 years of MDS experience; skilled nursing background preferred.
• Strong communication skills, proactive approach, and ability to anticipate needs.
• Ability to foster a professional, respectful, and caring environment.
