Job Schedule: Part time, 12 hour shift, every other weekend rotation, extra night differential.

Job SummaryThe Licensed Practical Nurse (LPN) provides professional nursing care for residents of all ages in long term care, under the supervision of a registered nurse, advanced practice provider, or physician. Demonstrates knowledge of nursing principles for the resident in the relevant clinical setting. Demonstrates competency and practices within the full scope of nursing expertise/knowledge and utilizes appropriate age and population specific standards related to the physical and psychosocial needs of the resident as per care plan.
Possess the ability to perform testing to evaluate data reflective of the resident's status and to provide the care needed. Demonstrates ability to recognize problems, recommend solutions and respond effectively to assist in an emergent situation.
- Ensures the health, comfort and safety of residents.
- Contributes to the assessment of residents and administers medications or treatments as ordered.
- Provides technical support to healthcare professionals as needed.
- Documents resident interactions and outcomes related to resident care, medication, behavior, activity, and dietary.
- Skills in establishing and maintaining effective working relationships with resident, health care providers, and the public.
- Communicates clearly and maintains confidentiality of sensitive information.
- Counsels residents, family members and/or resident advocate on prevention and treatment plans.
- Performs other related duties as assigned.
- Graduate from an accredited practical nursing program as a Licensed Practical Nurse;
Associate degree preferred. - Graduation from a Board approved AASPN program with IV therapy or completion of approved IV Therapy course (if required). Must have attended a pre-licensure nursing program with supervised clinical experience across the lifespan or a minimum of 400 hours of experience as LPN at another facility.
- Currently holds an unencumbered Licensed Practical Nurse (LPN) license with State Nursing board and/or possess multi-state licensure privileges.
- Obtains and subsequently maintains required department specific competencies and certifications, completes required annual continuing education units (CEU) and competency training requirements.
