The Licensed Practical Nurse (LPN/LVN) provides and documents skilled nursing care under the supervision of a Registered Nurse, following the patient’s care plan and physician orders while maintaining confidentiality and professional boundaries.
Essential Job Functions- Deliver and coordinate patient care in compliance with physician orders.
- Continuously observe and assess patient condition and report changes.
- Document all activities, assessments, nursing actions, and care coordination timely.
- Participate in the development, implementation, and update of the nursing care plan.
- Take appropriate nursing actions based on assessment to achieve expected outcomes.
- Implement interventions to prevent or reduce risk.
- Comply with Aveanna policies, state and federal regulations, accrediting bodies, and the Nurse Practice Act.
- Apply infection control measures consistent with OSHA standards.
- Educate patients and families on disease processes, self‑care, and prevention.
- Maintain competency through educational programs, continuing education units, and skill evaluations.
- Graduate of an accredited nursing school.
- Current, unrestricted state license as an LPN/LVN.
- Current CPR certification.
- Proficiency in clinical assessments, documentation, and compliance with nursing policies.
- South Carolina – One year of pediatrics experience.
- California – One year of experience under a current nursing license.
- Louisiana – One year of experience as a licensed nurse.
- Continuing education as required by state.
- Six months of recent experience in a clinical care setting.
- Home health experience.
- Attention to detail.
- Time management.
- Effective problem‑solving and conflict resolution.
- Good organization and communication skills.
- Speak, write, read, and understand English.
- Travel to client sites.
- Lift up to 50 pounds.
- Reposition patients and move equipment without assistance.
- Prolonged walking, standing, bending, kneeling, reaching, twisting.
- Sit and climb stairs.
- Visual and hearing acuity.
- Strong sense of smell and touch.
- Respond physically and mentally to emergencies.
- Work in diverse environments that may involve allergens and other conditions.
- Possible exposure to blood, bodily fluids, and infectious diseases.
