Qualifications
  • Active, unencumbered LPN/LVN license in the state of practice
  • Compact licenses transferred to residence state within 90 days
  • Current CPR certification (hands‑on component)
  • Current TB skin test (within last 12 months)
  • Six months of hands‑on nursing experience preferred but not required
  • Reliable transportation
Position Overview

The Licensed Practical Nurse (LPN/LVN) provides and documents skilled nursing care under the supervision of a Registered Nurse, following the patient’s care plan and physician orders while maintaining confidentiality and professional boundaries.

Essential Job Functions
  • Deliver and coordinate patient care in compliance with physician orders.
  • Continuously observe and assess patient condition and report changes.
  • Document all activities, assessments, nursing actions, and care coordination timely.
  • Participate in the development, implementation, and update of the nursing care plan.
  • Take appropriate nursing actions based on assessment to achieve expected outcomes.
  • Implement interventions to prevent or reduce risk.
  • Comply with Aveanna policies, state and federal regulations, accrediting bodies, and the Nurse Practice Act.
  • Apply infection control measures consistent with OSHA standards.
  • Educate patients and families on disease processes, self‑care, and prevention.
  • Maintain competency through educational programs, continuing education units, and skill evaluations.
Requirements
  • Graduate of an accredited nursing school.
  • Current, unrestricted state license as an LPN/LVN.
  • Current CPR certification.
  • Proficiency in clinical assessments, documentation, and compliance with nursing policies.
Additional State‑Specific Requirements
  • South Carolina – One year of pediatrics experience.
  • California – One year of experience under a current nursing license.
  • Louisiana – One year of experience as a licensed nurse.
  • Continuing education as required by state.
Preferences
  • Six months of recent experience in a clinical care setting.
  • Home health experience.

Physical Requirements
  • Speak, write, read, and understand English.
  • Travel to client sites.

  • Respond physically and mentally to emergencies.
