Certified Nurse Assistant
- Long Term Care POSITION

DESCRIPTION:

The Certified Nurse Assistant (CNA) provides services to assigned residents in accordance with care plans, facility policies and procedures and at the direction of supervisor(s).

Supervisory Responsibilities:

None Duties/Responsibilities:

Provides supportive services to nurse(s) and other staff as needed and performs duties as assigned.

Assists residents with or performs activities of daily living for residents in accordance with care plans and established policies and procedures.

Assists resident with lifting, turning, moving, positioning, and transporting into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc.

Coordinates dining room services at assigned mealtimes, including set-up and clean-up, meal tray delivery, feeding assistance, and documentation of meal intake.

Delivers nutritional supplements to residents at assigned times and provides assistance as necessary to ensure intake. Documents intake accordingly.

Assists with weighing residents according to facility policy, and records weight in designated locations. Reports weight changes to nurse and supervisor.

Assists with tracking the condition of the resident’s skin. Reports any presence of pressure areas, skin breakdown or skin tears to nurse and supervisor.

Assists nursing staff in carrying out toileting program activities.

Completes flow sheets daily to indicate that the specified task was done. Documents the progress of each resident as directed by supervisor.

Keeps nurse and supervisor informed of factors that interfere with being able to perform the work as assigned (i.e. resident refusal, reassigned to nursing unit).Reports change in a resident’s condition to the resident’s nurse and supervisor.

Maintains established facility policies and procedures, objectives, performance improvement program, safety, environmental and infection control standards, and protocols.

Maintains a clean, safe, comfortable, and therapeutic environment for patients and families in accordance with hospital standards.

Provides general aspects of care that meet psychosocial or physical needs or both simultaneously, within the nursing assistant’s scope of practice.

Meets communication needs of patients.

Treats patients and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic, and religious/spiritual needs of patients and family.

Demonstrates performance reflecting the initiative and responsibility expected of a nursing assistant.

Assists in admission, transfer, and discharge of patients.

Able to communicate effectively in English, both verbally and in writing.

Maintains confidentiality of protected health information, including verbal, written, and electronic communications.

Assist with orientation of new employees

Use proper lifting and body mechanics while delivering care to patients and meet the physical demands of the position.

Reports changes in a patient’s condition to the patient’s nurse and/or supervisor.

Maintains a working knowledge of all equipment used in treatment of patients

Maintains and replaces supplies on unit assigned.

As a condition of employment, completes all assigned training and skills competency.

Performs other duties as assigned, within the Nursing Assistant’s scope of practice.

Physical Requirements:

Physically demanding, high-stress environment

Exposure to blood and body fluids, communicable diseases, chemicals, radiation, and repetitive motions

Standing and walking for extensive periods of time Ability to assist in evacuation of residents during emergency situations.

Ability to bend, stoop, kneel, crouch, perform overhead lifting and perform other common physical movements as needed for the position.

Dexterity of hands and fingers to perform resident care.

Ability to lift, move, push or pull a minimum of 35 pounds.
