Location: Beulah

Nurse for Days/evenings to start as soon as possible; it would be a 36hr GTE. A mix of 8,12 and 16 hours, with possible overtime; some weeks they would work 3-5 days a week depending on the week. Mainly weekends also.

Responsibilities

Provide licensed practical nurse care to residents in LTC settings, following care plans.

Assess resident conditions, document outcomes, and flag changes to RN/MD promptly.

Administer medications and treatments per physician orders and facility policies.

Ensure compliance with regulatory requirements, safety standards, and facility policies.

Collaborate with interdisciplinary team to ensure quality care and adherence to protocols.

Monitor and manage vital signs; respond to emergencies following code procedures.

Maintain accurate, timely documentation to support performance metrics and audits.

Meet daily targets for care delivery and contribute to pipeline-based planning.

Document and address patient safety concerns, including fall prevention and infection control.

Deliver patient/family education on care plans, medications, and safety measures.

Participate in ongoing professional development and demonstrate readiness for advancement.

Support onboarding of new staff and mentor peers to promote team productivity.

Experience/Qualifications

Licensed Practical Nurse (LPN) license valid in state of practice.

Experience in long-term care (LTC) or senior care settings preferred.

Administer medications and treatments per physician orders and facility policies.

Accurate documentation and EMR/charting with attention to metrics and audits.

Compliance with regulatory standards (e.g., state/federal LTC regs) and safety protocols.

Proficiency in vital signs monitoring and rapid response to emergencies.

Strong teamwork with interdisciplinary staff and ability to mentor peers.

Patient and family education on care plans, medications, and safety.

Ability to meet daily care targets and contribute to pipeline-based planning.

Onsite availability for contract role with commitment to growth and professional development.
