Location: Garrison
CHI St. Alexius Health Garrison seeks a Social Worker to proactively assess, develop, and implement care plans with patients, families, and an interdisciplinary team across Garrison and Turtle Lake facilities.
Daily duties include psychosocial assessments, patient interviews, and providing therapeutic care to help patients achieve treatment goals appropriate to their age. Strong focus on care coordination, advocacy, and community resource navigation.
